胃癌會遺傳嗎？

提起拿破崙，恐怕沒有幾個人不知道，但您可能不知道，他的祖父、父親、1 個兄弟以及 3 個姐妹都因胃癌去世，整個拿破崙家族包括他本人在內，共有 7 人患胃癌。難道胃癌也會遺傳？

胃癌確實存在遺傳的可能

在歐洲及北美，約有 3% 的胃癌呈現家族聚集的現象，其中在義大利有 8%～10% 的胃癌呈現家族聚集傾向，並且，與正常人相比，胃癌患者的直系親屬患胃癌的風險升高約 3 倍^[1]。在一些亞洲國家，例如日本、韓國，分別有 0.9% 和 0.5%～0.8% 的胃癌呈現家族聚集傾向^[1]。

胃癌在家族中聚集的現象除了與家庭成員處於相同的生活環境、具有相似的飲食習慣或與某些偶然因素有關外，還可能與遺傳有關。已有的觀點認為，胃癌可以被看作是具有腫瘤遺傳傾向的腫瘤綜合徵的一部分，也就是說，胃癌可能是具有遺傳性的腫瘤綜合徵的表現之一^[2][3][4]。

胃癌的遺傳與哪些因素有關？

如何理解胃癌的遺傳性？與遺傳因素相關的胃癌即遺傳性胃癌，約佔胃癌的 1%～3%^[5][6][7]，其中最常見的就是遺傳性瀰漫型胃癌。據報告，遺傳性瀰漫型胃癌跟以下一些基因突變有關：

• CDh2 基因，被認為是遺傳性瀰漫型胃癌的危險因素和標誌之一^[8]。

• 編碼 α-E-連環蛋白的 CTNNA1 基因突變，也可能導致遺傳性瀰漫型胃癌的發生^[9]。
• MAP3K6、BRCA2、PALB2 基因等也可能與遺傳性瀰漫型胃癌有關^[10][11]。

確實，胃癌可能具有遺傳性，與一些基因的突變有關，但更多患者的胃癌是後天多種因素綜合作用的結果。因此，建立良好的飲食習慣，創造良好的生活環境，避免或儘早消除幽門螺桿菌感染等，對於預防胃癌的發生可能更關鍵。
